Beginning February 1, we will exclusively be using Final Forms for athletic physicals. Paper copies will no longer be accepted. All parents/guardians will receive an email from Final Forms to set up an account on February 1.
What if my student's athletic physical is up to date?
If your student athlete has an athletic physical that is up to date, it would be wise to set up your account but no need to upload the physical copy of the paperwork, we have those on file in the office. After February 1, when your athlete's physical expires, you will transition fully to Final Forms.
What if my student's athletic physical has expired or is about to expire?
Please exclusively use Final Forms. Almost all of the documents are now electronic and only require an electronic signature, it is very user friendly. You will only need to upload the documents that the physician signs. Once you, as the parent/guardian, have completed the forms, Final Forms will automatically send an email to the athlete for their electronic signatures on the required documents.
I have completed everything and my account says it needs to be reviewed?
No need to take any action on that. The athletic trainer will review each account, once completed, the status will automatically change.
Parents and athletes who have set this up reported that it took them about 15 minutes total.
